- First Name
- Last Name
- Nickname
- AIM screen name
- Yahoo IM screen name
- Jabber/Google Talk user name

Download the file and put it in your plugins directory.
Activate it.
To configure what fields are required register an account, login to the WordPress Dashboard. Then go to “Options” > “Force User Field Registration”
